To submit a request for assistance, click the Submit a Request link at the top of any page in the Help Center. Fill out the new request form and click the submit button. Below is a description of each field required to be filled out:
NOTE: This information will be emailed to you once submitted, so be sure not to include the full SSN or any other personally identifiable information (PII) if your issue is regarding a specific student. Instead use the student ID, first and last name, or other information that will allow us to identify the student.
CC
Here you can add the email addresses of anyone you wish to cc on the ticket.
Subject
Enter a brief description of the subject. This will be the subject of the email notifications you receive from FAME.
Description
Enter as much detail about the question or issues as possible. The more information you give to the FAME Representative, the easier it is to help resolve your issue.
Alt. Phone Number
If you would prefer to be contacted at a number other than the campus number we have on file, enter in the best number to reach you.
Title IV Compliance Issue?
If your issue can potentially affect your Title IV compliance, choose the option that best fits your issue. Otherwise, choose "No".
School Campus
Choose the campus for which this issue pertains. If no campus is displayed or you do not see the correct campus, then you must call FAME Support at 1-800-327-5772 for assistance. It is possible that your email address is not registered.
FAME Product/Service
Choose the product or service pertaining to your issue. If you do not see the correct product or service, then you must call FAME Support at 1-800-327-5772 for assistance. Note: Only products or services that are "on-service" with FAME will be displayed.
The following fields will be populated automatically based on your campus selection:
- FAME Account ID
- KISS ID
- Financial Aid ID
- City
- State
- Campus Phone Number
